This foundational paper by the Task Force of the European Society of Cardiology and the North American Society of Pacing and Electrophysiology established the standard methods for measuring HRV and defined what different HRV metrics mean physiologically. It confirmed that reduced HRV is associated with increased risk of mortality following myocardial infarction, and remains the most widely cited reference in HRV research worldwide.
What this means for you
HRV is not a fitness metric, it is a window into how well your nervous system is regulating your body. A consistently low or declining HRV over time is worth paying attention to, even if you feel fine.
HRV and inflammation
C-Reactive Protein Is Associated with Heart Rate Variability

This study of 269 patients found a clear and independent inverse relationship between CRP (a key marker of systemic inflammation) and HRV indices including SDNN and SDANN. Patients with higher CRP levels had significantly lower HRV, and the association held after adjusting for age, smoking, medication, and cardiovascular history. The authors concluded that low-grade inflammation and reduced autonomic function are independently linked.
What this means for you
When your HRV is chronically low, it may not just mean you are stressed or under-recovered, it may reflect something deeper in your body's inflammatory state. Continuous monitoring makes it possible to spot these patterns before they become symptoms.
HRV, resting heart rate, and subclinical inflammation
Increased Heart Rate and Reduced Heart-Rate Variability Are Associated with Subclinical Inflammation in Middle-Aged and Elderly Subjects with No Apparent Heart Disease

This study of 678 adults without known heart disease found that both elevated resting heart rate and reduced HRV were independently associated with higher levels of inflammatory markers including CRP and interleukin-6. Crucially, this association existed even in people with no diagnosed cardiovascular condition, suggesting these metrics can detect subclinical inflammation years before symptoms emerge.
What this means for you
You do not need a heart condition for HRV to carry meaningful health signals. In apparently healthy middle-aged adults, low HRV and elevated resting heart rate together predicted hidden inflammation. Tracking both continuously gives you an early-warning system most people do not have.

RHR — Resting Heart Rate
Resting heart rate is the number of times your heart beats per minute when you are at complete rest. It is one of the most accessible and well-studied cardiovascular markers available.
RHR as a long-term predictor of cardiovascular risk
Elevated Resting Heart Rate, Physical Fitness and All-Cause Mortality: A 16-Year Follow-Up in the Copenhagen Male Study

This 16-year follow-up of 2,798 men found that elevated resting heart rate was a strong and independent predictor of all-cause mortality, cardiovascular mortality, and sudden cardiac death. Importantly, the association held even after adjusting for physical fitness meaning a high resting heart rate carried its own risk regardless of how fit the individual was. Men with an RHR above 90 bpm had roughly double the mortality risk of those below 50 bpm.
What this means for you
Being fit does not fully cancel out a high resting heart rate both matter independently. Tracking RHR continuously, rather than checking it occasionally at a GP appointment, gives you a far more accurate picture of your baseline and how it shifts over time.
RHR and major cardiovascular events
Heart Rate as a Risk Factor for Cardiovascular Disease Prevention: The Emerging Evidence

This review synthesised evidence from multiple large cohort studies and found consistent evidence across populations that an elevated resting heart rate predicts major cardiovascular events including myocardial infarction, heart failure, and stroke. The authors noted that resting heart rate is easily measured, requires no specialist equipment, and provides prognostic information beyond traditional risk factors such as blood pressure and cholesterol.
What this means for you
Your resting heart rate is not just a fitness number it is a cardiovascular risk marker that is simple to measure and consistently meaningful across large populations. The direction your RHR trends over weeks and months tells you more than any single reading.
RHR across 30 studies: a meta-analysis
Prognostic Value of the Resting Heart Rate: A Meta-Analysis

This meta-analysis reviewed data from 30 studies and found that resting heart rate was a consistent independent predictor of cardiovascular mortality in both men and women, across healthy populations and those with existing disease. Each 10 bpm increase in resting heart rate was associated with a meaningfully increased risk of cardiovascular death. The authors called for resting heart rate to be routinely assessed as part of cardiovascular risk profiling.
What this means for you
The pattern here is consistent across dozens of studies: your resting heart rate predicts your cardiovascular risk. Most people check it once a year at a GP visit continuous overnight monitoring gives you a daily picture that is far more informative.

Blood Pressure
Blood pressure is one of the most important cardiovascular markers and one of the most undertreated, partly because it is rarely measured at the right time or with enough frequency.
The problem with clinic-only measurement
Masked Hypertension

This clinical review confirmed that masked hypertension where blood pressure appears normal in a clinical setting but is elevated in daily life — affects between 10% and 30% of the general population. People with masked hypertension face the same cardiovascular event risk as those with sustained hypertension, yet are routinely missed by standard clinic-based measurement. The review identified ambulatory monitoring as essential for detection.
What this means for you
A single reading at a GP appointment captures one moment in time. If your blood pressure rises during stress, poor sleep, or exertion and drops back by the time you see a doctor, the problem stays invisible. Continuous monitoring changes what is possible to detect.
Blood pressure variability as an independent risk factor

This analysis found that variability in blood pressure readings over time, independent of average blood pressure level, was a significant predictor of coronary heart disease, stroke, and heart failure. Patients whose blood pressure fluctuated widely between visits had elevated cardiovascular risk even when their average reading was within a normal range. The finding highlighted that consistency of blood pressure matters as much as its absolute level.
What this means for you
It is not just about what your blood pressure reads today. How much it fluctuates day to day and week to week carries its own risk and you cannot see that variability from occasional readings. You need a continuous or near-continuous picture.
Nocturnal blood pressure and cardiovascular risk

This landmark study examined nocturnal blood pressure dipping patterns in a large cohort and found that non-dippers people whose blood pressure does not fall adequately during sleep — had significantly higher rates of cardiovascular events compared to dippers, regardless of their daytime blood pressure levels. The study established the nocturnal dipping pattern as an independent cardiovascular risk marker.
What this means for you
What your blood pressure does while you sleep may matter more than what it does while you are awake. Most people have no idea whether they are a dipper or a non-dipper continuous overnight monitoring is the only practical way to find out.
